I actually know the fix for this. This happens if you have the automatic skill check option enabled as you’re using a controller. It automatically hits the skill check and when you try to vault after the skill check is when you can’t vault anything entirely. You have to press the START button and press it again and you should be able to vault again with your controller. This usually always works for me and I’ve noticed it only happens when you receive a skill check and you have the automatic skill check ON.
